The above named Individual &/or Company hereby requests permission to produce works, on a limited basis, from Wardell Publication Inc. ©Copyrighted patterns and/or designs and offer them for commercial resale and by the signature below agrees to abide by both of the following limitations:

That the production quantity does not exceed 15 total accumulative pieces from any single individual ©copyrighted Wardell Publication Inc pattern and/or design and;

That the total sales revenue, either collectively or individually, of all product constructed from any single individual ©copyrighted Wardell Publication Inc. pattern and/or design, does not exceed $1500.00 in US funds (or equivalent in other currency).

It is also understood and agreed that this permission is canceled and revoked immediately upon the occurrence that either of the above provisions is exceeded. Upon this revocation the above named agrees to notify Wardell Publication Inc. immediately and to make an application for a commercial production license which may be subject to a royalty fee (royalty amount to be determined at the time of license application) payable to Wardell Publications Inc. for any production beyond the above stated limits.

Further, it is understood and agreed that this permission does not extend to the copying or reproduction of any pattern (except to make a fabrication pattern copy), drawing or photograph, contained in any ©copyrighted book. The reproduction in whole or in part of a Wardell Publications Inc. book is strictly prohibited for any reason including, but not limited to, advertising, promotions, brochures, catalogs, etc., either printed or broadcast, or by any other means either electronic, mechanical or otherwise, without permission in writing from Wardell Publications Inc., the copyright owner.

Note: Permission is not granted or implied without the original signature below of both the applicant and an official of Wardell Publications Inc.
